Please use this page to submit the new International Customer Agreement form. This form is also used to authorize additional countries
or change the countries you've previously authorized.
Important: Please note that each time you submit an updated International Customer Agreement, the "authorized" countries indicated on the form you submit (whether by mail, fax or online)
replace all previous authorizations. You should therefore be careful to clearly indicate ALL of the countries you want to authorize, even if you have previously done so.
Making an authorization does not mean that you will automatically receive medications from that country.
You always have the right to specify which country your order will be filled from.
