IF you have ordered from us previously and want to:
  • Place a refill order
  • Submit a brand new or "replacement" prescription or
  • Place a refill order AND submit a brand new or replacement prescription

limited power of attorney form

Please use this Limited Power of Attorney form to submit a Designation of Personal Representative. This allows someone else, such as a son or daughter, to act on your behalf in placing orders. It also gives Canadian Meds USA permission to discuss your personal health information with that person.

Please read this important information:

A replacement prescription is a newly issued prescription for a drug previously ordered (no change in dosage or strength). In most cases, a replacement prescription is submitted because all of the refills on the previous prescription have been used up. However, replacement prescriptions are treated the same as brand new prescriptions in terms of how they are processed by TCDS. Accordingly, when either a brand new or replacement prescription is being submitted, you should allow approximately 10 to 21 days for your order to be delivered. Refills not involving a brand new or replacement prescription are normally delivered within 10 to 14 days.
